This is a factory Fostech Orgin 12 Short Barrel Shotgun. This model has the Nickle Receiver as well as Nickle Internals.

It comes with a hard case, two 5 round magazines, one 10 round magazine, a choke adapter for a SiCo Salvo 12, extended charging handle, and a belt mounted mag carrier. The 30 round drum is available for an additional cost.
